Baptism RegistrationTo register your infant to be baptized at our parish, please complete this form and submit it to the address above. If you have any questions, please contact our parish office.

Baptism Preparation ClassParents and godparents are required to attend a Baptism Preparation Class offered on one Sunday of each month in the Parish Activity Center. To register for our next class, please complete the form below and return it to our Parish Office.Please email it to Susan Ozowski at [email protected]. Baptismal Certificates are available upon request. Please include the child's name, whether baptized or to be baptized, in any email you send to our staff.
